Job Requirements/Description

We are seeking a Community Nurse to join our team.

You will be part of a team that provides a safe, effective, patient centered & evidence-based service in the community setting, delivering nursing care to patients across our pathways including Planned nursing, Urgent Care Response, Planned therapy & hospital discharges.

You will be working within adult services to deliver a full range of clinical activities in both acute & long term care, supported by a robust competency framework to care for adults within their own home & other settings.

You will have opportunities to utilise, learn or develop your clinical skills such as, wound care, catheter care, end of life care, venepuncture & medication administration.

You will be working as part of an integrated multi-disciplinary team (MDT), which includes:

  • Advanced Clinical Practitioners - Long Term Conditions
  • Advanced Clinical Practitioners - Urgent
  • Clinical Lead Therapists
  • Community Nurses
  • Occupational Therapists and Physiotherapists
  • Nurse Associates, Assistant Practitioners
  • Community Support Workers
  • Administrative staff
  • Practice Education Facilitators
  • Operational Managers

Main duties of the job

  • To use clinical reasoning skills to assess, plan, implement & evaluate patient-centred care using an evidence based approach, promoting high quality, harm free care. Collaborate with service users, carers & families to identify appropriate & achievable goals offering education & promoting independence.
  • To work within a multi-disciplinary team, providing integrated skills & sharing knowledge. Ensure effective & appropriate communication with staff in primary care, secondary care & external agencies.
  • To maintain accurate information & legal records of all interventions in accordance with Sirona Care & Health Policy & The Nursing & Midwifery Council's (NMC) Code of Conduct.
  • To actively participate in training & develop additional competencies required for the role.
  • To work within your scope of practice, being responsible for your own delegated objectives, training, proficiencies & competence as outlined by the NMC Code of Conduct.

Shortlisting criteria

Essential

  • BSc or equivalent experience in Adult Nursing.
  • Current or pending NMC registration.

Essential Criteria

  • Clear understanding of NMC standards of proficiencies and professional standards of practice including revalidation.
  • Effective communication skills, written, verbal and non-verbal with the ability to adapt communication for individuals with communication difficulties.
  • Ability to work effectively autonomously and as a team member. Ability to make clinically reasoned judgements when working with a variety of conditions, knowing when to seek appropriate advice / guidance from senior staff.
  • Knowledge of the principles of Clinical Governance and its application.
  • Willingness to undertake necessary training.
  • Competent IT skills.

Desirable

  • Community work (either student or post graduate)
  • Experience and understanding of the application of health, safety and risk management policies.
  • Working with a range of professionals in Health or Social Care.
  • Up to date knowledge of Evidence Based Practice, local and national standards.
