Job Requirements/Description

Do you have a passion for urgent care? Would you like to make a difference to your local community?

We are seeking an Advanced Clinical Practitioner to join our Urgent Community Response (UCR) pathway in South Bristol!

The successful applicant will be working as an Advanced Clinical Practitioner supporting the Urgent Community Response pathway to reduce unnecessary hospital admissions and contribute to effective admission avoidance.

The role incorporates high-level clinical assessment skills and use of the non-medical prescribing qualification to medically support complex patients within the community, presenting with a wide range of acute medical conditions.

Due to the qualification criteria for this role requiring a Non-Medical Independent & Supplementary Prescriber (or willingness/ability to work towards) , we are currently unable to accept applications from Occupational Therapists.

Main duties of the job

As an ACP, you will work autonomously as part of the Community Urgent Response pathway, part of our community Integrated Neighbourhood Teams.

The role involves effective history taking, clinical assessment, diagnosis, and the implementation of a range of interventions for patients presenting with acute undifferentiated and undiagnosed conditions and acute management of chronic long-term illnesses.

The successful ACP will need to demonstrate critical thinking in the clinical decision-making process; identify gaps within the service and assist with service improvement and development in line with national and local strategies.

You will be required to work collaboratively with other members of the multidisciplinary team to establish a whole systems approach to managing health care for individuals with complex care needs.

The post holder will provide clinical leadership and support the delivery of effective and safe clinical standards of practice within a variety of community services.

Job description

Job responsibilities

This post is not an Advanced Practice role that is mapped to the HEE Multi-professional Framework for Advancing Practice. This is because these roles do not require candidates to work across all 4 pillars of Advanced Practice to MSc level and are therefore banded at AFC Band 7. They do, however require a skillset working at or towards MSc level in the Advanced Practice Clinical Pillar. This role will not be eligible for training funding for an MSc in Advanced Practice however training may be supported depending on individual learning needs identified following Sirona CPD funding application processes.

Sirona care & health currently recruit overseas nursing and therapy staff through an NHS Trust Supplier and we adhere to the NHSE Code of Practice in relation to applicants from Red List countries. We are therefore, unable to accept any direct applications to this advert from candidates who are not already based in the UK.
