Stroke Sub Acute Rehabilitation Unit

Our Stroke Sub Acute Rehabilitation Unit (SSARU) is a community-based unit which meets the needs of people who are unable to return home immediately after a hospital stay. The unit consists of 30 beds at South Bristol Community Hospital, which provides a safe environment where individuals who are 'medically stable' can receive Stroke specialist rehabilitation assessment, along with focused therapy and support from our multidisciplinary team, to support independence before they return home. Once the person arrives home, their stroke care is handed over to our Integrated Community Stroke Service (ICSS) at Home team. All patients on the unit will also be under the care of a Stroke Consultant who will support with weekly board rounds and remote advice when required.

Main duties of the job

  • Provide senior oversight for the medical input for the cohort of inpatient service users within the ward.
  • Use broad medical knowledge to manage a range of clinical presentations that will sit outside of the regular cohort of conditions that are routinely managed within the Advanced Practice scope of practice and conditions regularly seen in primary care - including and not limited to; skin disease, rheumatological conditions and management of heart failure.
  • Use a holistic approach to ensure factors within the Biopsychosocial model are considered
  • To consider whether there may be any reservable factors contributing to presenting conditions, including a thorough review of medication, in conjunction with pharmacy colleagues.
  • Medicines optimization and de-prescribing to reduce burden of medication for older people.
  • Initiating further investigations for abnormal blood tests, ordering radiology interventions including X-ray, USS and CT scans.
  • Referring, if clinically indicated via 2 week wait for suspected cancer and referring or liaising with other specialist services within secondary care and specialist services.

  • Higher Degree or Diploma of Geriatric Medicine or similar

Knowledge, Skills & Experience

  • Experience of working with older people and ideally hold a relevant Higher Degree/Diploma of Geriatric Medicine or similar
  • Experience of assessing and treating patients in a community setting
  • Experience of working effectively as part of a multidisciplinary team
  • Ability to communicate effectively with patients, their families and colleagues
  • Ability to appraise research critically and apply research outcomes to clinical problems
  • Ability to assess and treat elderly patients

Desirable

  • Experience in teaching and motivating/implementing change
  • Experience of supervising ACPs / teaching
